Healthcare providers can submit referrals using either of these methods. We do not accept referrals by non-secure email to protect patient privacy.

What to Include in a Referral
- Complete patient demographics (name, date of birth, contact information)
- Insurance information
- Reason for referral and primary diagnosis
- Relevant medical history and current medications
- Recent test results or imaging (if applicable)
